Carolina Canines for Service serves the southeastern region of the United States, from Virginia to Florida and east of the Mississippi River. If you are located outside of this area, we cannot accept an application at this time. Recipients of our service dogs from outside a 150 mile radius of Wilmington, NC must travel to Wilmington at their own expense for 1-2 weeks training with their Carolina Canine Service Dog. This team training is the key to forming a life-long bond between human and dog and for the recipient to learn how to utilize his/her canine partners skills.

If you have a disability and feel you could benefit from the assistance of a Service Dog, please click here to print an application or contact us to request an application packet by calling 866-910-3647 or email [email protected]

Please note: A $25 application fee is required at the time of submission of application.
